MyMDHub is a digital health platform that helps users upload, organize, and share records, connect Medicare data through CMS Blue Button 2.0, and interact with participating pharmacies, providers, and clinical programs within MDHIN.

2. No Medical Advice

MyMDHub does not provide medical advice, diagnosis, or treatment. Clinical services available through the platform are provided by licensed healthcare professionals. Always consult an appropriate healthcare professional for medical guidance.

3. Eligibility

By using the platform, you represent that you are at least 18 years old, legally authorized to use healthcare services in your jurisdiction, and providing accurate information.

4. Blue Button Access

By connecting your Medicare account, you authorize MyMDHub to access the data you approve through CMS Blue Button 2.0. You may revoke access later.

5. Account Responsibilities

You are responsible for safeguarding your login credentials, reporting unauthorized access, and maintaining accurate account information.

6. Payments and Programs

Certain programs offered through MDHIN may require payment. Program terms, pricing, and subscription details will be disclosed before enrollment. Some programs may involve compounded medications prescribed and dispensed in accordance with applicable law.

7. Provider and Pharmacy Relationships

MyMDHub helps facilitate interactions with pharmacies, providers, and service partners that operate independently and are responsible for the services they provide.

8. Privacy

Your use of the platform is also governed by the Privacy Policy available at /privacy/.

9. Limitation of Liability

To the fullest extent permitted by law, MyMDHub is not liable for indirect, incidental, or consequential damages, does not guarantee uninterrupted operation, and is not responsible for outcomes from independent provider or pharmacy services.

10. Termination

We may suspend or terminate access for misuse, fraud, violation of these terms, or legal compliance reasons.

11. Modifications

We may revise these Terms from time to time. Continued use of the platform after changes are posted constitutes acceptance of the revised Terms.
